The Annual  Wellness And Care System gathers over 100 different personal health information data points during an initial telephone assessment and subsequent visit to the Provider.  The data is analyzed automatically to generate a set of risk factors for each Patient.  The Provider reviews the risk factors and develops follow-up care initiatives based on the data.  A separate report is available to give to the Patient describing their personal risk factors.
